Home Instead Senior Care Medication RemindersReturning Home Post-Discharge Care
When a senior transitions home after a hospital or facility stay, common issues such as mistakes made in medications, missed follow-up medical appointments, poor nutrition and dehydration, and lack of safety preparation in the home can often lead to readmission.

The Home Instead Senior Care network’s returning home post-discharge care services help ensure a senior’s successful recovery and reduce the likelihood of readmission. Home Instead CAREGivers can assist with discharge coordination and execution, medication reminders, nutrition management, warning sign monitoring and notification, and record keeping. If you have a patient who could use immediate assistance upon discharge, refer them to your local Home Instead Senior Care office.

Practitioner's Tip
When the Stress Rises, Deepen Your Breath
Take a moment right now to notice how you’re breathing. Is your inhale short and shallow, causing only your chest and shoulders to rise? If so, that may be a sign that you need to relax. On your next breath in, allow your abdomen to expand as your lungs inflate fully. Hold the air there for a few seconds before exhaling. Then, feel your tension ease as you breath out slowly. Repeat this cycle of deep breathing several times in a row and notice how you feel afterward. Anytime throughout the day when you catch yourself feeling stressed or overwhelmed by the demands of your patients, your colleagues or your family members, you can return to that feeling of calmness by breathing deeply.
